What it uses
Vermont has three statewide '813' affidavits split by case type: 400-00813A (income and expenses) plus 400-00813B (property and assets) for divorce-type cases, and 400-00813S for parentage only.
Which form, when
Case type, not income. 813A is required in a new divorce, civil union dissolution, legal separation, annulment or parentage case with minor children; in a motion to modify child support or maintenance; for a support obligor facing enforcement; when the child is in DCF custody; or on court order or a discovery request. 813B is required in a new divorce, dissolution, separation or annulment with minor children, or on order or request. 813S is required in a new parentage case, or on order or request. All per each form's printed instructions; the court page says 813A and 813B are 'used in divorce cases' and 813S is 'used in parentage cases only'.
Filed or exchanged
Filed with the court before or at the first case manager's conference, or at least 7 days before the first hearing if no conference is set, with a copy sent to the other party (and to OCS if involved) at the same time (form instructions; vtcourts.gov child-support page: 'file them with the court, and give a copy to the other party').

This is general information, not legal advice. Forms change, so check with the court before you file.
