Filed with the court, or given to the other side?

The disclosures go to the other side within 35 days after the response and are not filed; only the certificate of service is. The Affidavit Verifying Income is filed with a request for child support.

What it uses
Statewide forms split by issue: an Inventory of Property and Debts, an Affidavit Verifying Income for child support (with a disclosure cover sheet), and an affidavit for temporary orders/spousal maintenance; IRFLP 401 sets the disclosures.
Which form, when
IRFLP 401: property and debts -> Inventory (401(f)-(g)); child support -> Affidavit Verifying Income per Rule 120 and Form 5 plus worksheet and 3 years of returns (401(b)); spousal maintenance or fees -> affidavit with the Rule 504(a)(2) information (401(e)), i.e. IRFLP Form 3.
Filed or exchanged
Exchanged, not filed: within 35 days after a responsive pleading each party discloses in writing under oath (IRFLP 401(a)); the self-help divorce page says 'Only the Certificate of Service is filed with the court. Do Not File the Disclosures with the court.' The Affidavit Verifying Income is filed with a petition or motion seeking child support (IRFLP 504(a)(2)).

This is general information, not legal advice. Forms change, so check with the court before you file.
