What it uses
Two statewide AOC Financial Affidavits, Form 75 before the decree and Form 74 after it, each covering income, expenses, assets and liabilities; no rule found that makes either mandatory, and districts such as Tulsa publish their own declarations.
Which form, when
AOC Form 75 before the decree, AOC Form 74 after it (form titles; both cite 43 O.S. 118); no rule found making either mandatory. Local: Tulsa Rule DR 5 (amended eff. Aug 24, 2023) requires exchange of income, child-care, insurance, debt and living-expense statements without naming a form; Northeast Judicial Administrative District Rule 3.3 (not Tulsa) requires a Pre-Trial Financial Declaration in contested dissolutions.
Filed or exchanged
AOC forms are filed ('This document is filed by father/mother'); statewide, 43 O.S. 110(A)(1)(h) requires each party to deliver tax returns, two months of pay stubs, six months of bank statements, insurance, child-care and debt documents to the other within 30 days; Tulsa DR 5 statements are exchanged; NE Rule 3.3 declarations are served 5 days before trial and 'shall not be filed'.
