Montana financial forms for family court

Montana uses statewide self-help forms for property and debts and for income and expenses. One spouse filing alone uses MP-500 and MP-510; a joint petition uses MP-520 and MP-521.

Filed with the court, or given to the other side?

MP-500 is filed with the petition and served. MP-510 and MP-520 go only to the other spouse and are not filed unless the court orders it. MP-521 is filed.

What it uses
Statewide Montana Supreme Court/MLSA self-help forms split by issue (property and debts vs income and expenses) and by case type (individual petition MP-500/MP-510 vs joint petition MP-520/MP-521), all implementing the disclosure duty in MCA 40-4-252 to 254.
Which form, when
Individual dissolution (with or without children): MP-500 Proposed Property Distribution plus MP-510 Disclosure of Income and Expenses (MP-510-A/B if more lines are needed). Joint petition (MP-122 etc.): each spouse completes MP-520 Disclosure of Income and Expenses and the couple files one MP-521 Joint Proposed Property Distribution. Source: courts.mt.gov dissolution form pages and MP-122 para. 8-9; MCA 40-4-252 (preliminary declaration within 60 days of service).
Filed or exchanged
Mixed: MP-500 is filed with the petition and served (courts.mt.gov 'How to File' instructions, Step Two); MP-510 and MP-520 are served on the spouse only and not filed unless the court orders (form NOTE; MCA 40-4-252(2) 'may not be filed with the court, except on the court's order'); MP-521 is filed (MP-122 para. 8).

This is general information, not legal advice. Forms change, so check with the court before you file.
