Maryland financial forms for family court
Maryland uses one sworn financial statement in two versions (CC-DR-030 and CC-DR-031), chosen by the kind of support at issue. A joint property statement (CC-DR-033) is added when a marital property award is at issue.

- What it uses
- Two tiers of the same sworn financial statement (CC-DR-030 under Rule 9-203(b), CC-DR-031 under Rule 9-203(a)) chosen by the support issue, plus a separate joint property statement (CC-DR-033) whenever a marital-property award is at issue.
- Which form, when
- CC-DR-030 only when guideline child support is the only support issue and no party claims support outside the guidelines (Md. Rule 9-202(f)); otherwise CC-DR-031, which is also required whenever spousal support is claimed (Md. Rule 9-202(e)). The forms page puts it as combined income of $30,000 or less vs more than $30,000 (the guidelines schedule runs to $30,000 combined monthly income; the page omits 'monthly'). CC-DR-033 is required when a monetary award or other relief under Family Law Article 8-205 is an issue (Md. Rule 9-207(a)).
- Filed or exchanged
- Filed: CC-DR-030/031 are filed under affidavit with the pleading that makes or answers the support claim (Rule 9-202(e)-(f)); CC-DR-033 is filed jointly at least 10 days before trial or by the court's earlier date (Rule 9-207(c)).
